A Day At AgeWellHab Physical Therapy

Outpatient PT in the Home & Community — built for sustainability

No two days are identical—and that’s a good thing. Our clinicians have the autonomy to deliver high-quality care without the rushed pace of traditional models. Here’s what a typical day can look like.

​

Morning: Focused, High-Quality Visits

  • You decide if you have time to accept a patient into your  case-load

  • You will make your own schedule

  • You begin with a small set of visits planned with geography and flow in mind

  • Sessions emphasize mobility, balance, strength, gait safety, and confidence

  • You have time for real clinical reasoning, education, and progression—not just “getting through the visit”

What you’re not doing

  • No OASIS

  • No SOC/ROC pressure

  • No hospital-driven home health requirements

  • No double-booking

​

Midday: Documentation + Reset Time

​

  • Short documentation blocks built into the day- We partner with WebPT EMR

  • Space to return calls, coordinate care, or consult with your team

  • A sustainable pace that respects your attention and energy

  • Home exercise software for email, text or print to patients 

​

Afternoon: Continued Care + Long-Term Planning

  • Follow-ups focused on measurable functional goals

  • Clear communication with the supervising PT (for PTA visits)

  • If appropriate, you help patients transition into longer-term movement support through wellness options, so gains are maintained

​

Optional Pathway:

Community-Based Group Programming 

As GroupHab expands community partnerships, some clinicians may have the opportunity—if interested—to support future on-site programming such as:

In senior centers 

  • PT-led fall-prevention series (4–6 weeks)

  • Balance + strength group classes

  • Educational workshops (home safety, walking confidence, getting up from the floor)

  • Brief PT consult hours (when appropriate and aligned with center needs)

In senior living 

  • On-site small-group mobility & balance sessions

  • Wellness classes focused on strength, posture, walking endurance

  • Coordination with activities/wellness staff to support participation

  • When partnerships allow: convenient access to outpatient PT in residents’ living environment


These community programs depend on partnership development. We will always be clear about what’s currently active versus what’s planned.

End of Day: Leave Work Feeling Good

A day at AgeWellHab is designed so you can leave work thinking:
“I treated well. I educated well. I made progress with people who needed me.”  
Not: “I barely survived my schedule.”
